 Dear patients, friends and colleagues, We have so much to share with you this fall and winter. We are very excited to introduce you to Emily Kolenda Oehler, PA-C, who joined our practice in late September and replaced Sandra Elkhouly, PA-C, who relocated to Florida to be closer to her family. We will miss Sandra and we wish her well in all of her future endeavors. If you or your family members were seeing Sandra, please know that we will make sure there will be a smooth transition of your care to Shannon, Emily and me. Emily Oehler is a board certified physician assistant who graduated with honors from Butler University in 2001. There she earned a degree, Bachelors of Health Science in Physician Assistant studies. She joins us with seven years of experience from a busy cosmetic and general dermatology practice in Chicago. Emily currently is the President of the Illinois Society of Dermatology Physician Assistants (ISDPA). She also belongs to the American Academy of Physician Assistants (AAPA) and the Society of Dermatology Physician Assistants (SDPA). We welcome Emily to our practice.
In this issue we will talk about new things... the Matrix IR, a new non-invasive laser and radiofrequency device specifically for deeper wrinkles (for all of you who shy away from injectables or to complement them). We are also fortunate to be one of the first offices to get the VelaShape, the second generation of VelaSmooth that significantly decreases the number of treatments necessary to reduce cellulite and improve contour of the abdomen, legs and buttocks.
SKINQRI is heating up with many new studies such as Skinfo product studies, full face filler replacement study for moderate aging changes, and a study to determine the best strategy for reducing bruising from injectables.
In early October, I will be journeying to Buenos Aires for the World Congress of Dermatology where I will be presenting on the new Matrix IR technology and our findings from our rosacea research study. In mid-October the American Society of Dermatologic Surgery (ASDS) is here in Chicago where I am honored to participate in the pre-conference symposium for residents as well as present an abstract on the Fraxel SR1500 laser.
Of course by now you have noticed that Skinfo has moved downstairs to the retail level of CityPark, between Ticklebug and Lori's Shoes. We hope you will come and visit our new home, where it is much easier to peruse our expanded offerings. We know that you liked browsing while waiting for your appointments so please feel free to stop in on your way up and let them know to tell the front desk you are there. They will be happy to "check you in" and let you know when they are ready for you upstairs.
Finally, please pardon our dust as we remodel in the old Skinfo space to make an office for SKINQRI. This will probably result in some noise in the reception area for a couple of months,

 | Winterizing Your Skin Cold weather is just around the corner so now is the time to think about "winterizing" your skin! As the weather changes our skin changes right along with it and we need to often augment or alter our skin care regimen to facilitate the changes that occur. This could mean switching to a heavier moisturizer, or if acne prone, augmenting the moisturizer you use with a hyaluronic acid based gel. Alpha hydroxy moisturizers and treatment topicals can also help to exfoliate those dead skin cells that accumulate and cause our skin to look dull, so if you haven't been into Skinfo lately, consider updating or tweaking your current treatment plan now so you are ready for the cold winter months ahead.
Contact Dermatitis Contact Dermatitis is the term used when an individual comes in contact with an item they are allergic to. Nickel is one of the most common allergens, but there are hundreds of substances that could induce a contact allergy in an individual. Often the rash will occur in the shape of the item that touched the skin, i.e. a necklace that causes rash around the neck or a watch that causes a circular rash on the wrist or that belt buckle that constantly rubs the same area on the abdomen. Avoidance is the best treatment, but often prescription strength topical steroids are necessary to relieve the itching that occurs with the rash. On occasion an individual will need to undergo extensive patch testing to determine what the offending product may be. Hair Loss There are a number of types of hair loss. The non-scarring hair loss types consist of female/male pattern hair loss (androgenetic alopecia), telogen effluvium (stress hair loss) and alopecia areata.
· Androgenetic alopecia is the most common type of hair loss. In men it is called balding, and may be first manifested by a receding hairline or loss on the crown of the scalp. In women it typically shows up as a wider hair part, receding hair line or area of thinning on the top of the head in a circle starting just behind the hair line. The main cause of this is thought to be hormonal and genetic. The main treatments are minoxidil (Rogaine) and Propecia (oral medication approved for men only). Often this type of hair loss can be arrested and in some cases reversed by addition of one or both of these medications.
· Telogen Effluvium can be caused by childbirth, low thyroid, low iron storage, severe stress, and occasionally by certain drugs. Typically this type starts suddenly and the loss is all over, with increased shedding of hairs from the root. This usually goes on for approximately 3-6 months, then stops for 3 months, then the hair grows back. In the case of low thyroid or low iron, replacement to normal levels is necessary to restore hair growth.
· Alopecia Areata is an autoimmune disease, which means that the immune system attacks a portion of the hair follicle mistakenly thinking it is a bacteria or virus. This results in circular areas of complete hair loss. Sometimes there are broad patches or even loss of all hair on the head. This is treated with anti-inflammatory creams and injections as well as minoxidil.
PCOS Polycystic ovarian syndrome, or PCOS afflicts approximately 5-10% of childbearing women. Common symptoms of this syndrome include cystic acne often along the jawline and hirsutism, or excessive facial hair growth. In PCOS there is a hormonal imbalance that occurs with higher than average level of male hormones relative to female hormones. Other problems that can occur with this condition include insulin resistance (early diabetes) and high blood pressure, so it is important to discuss with your primary care physician or an endocrinologist to treat these more serious symptoms. We treat the associated breakouts and hair growth with various medications and also laser hair removal. For people with PCOS, we have found that laser hair removal will require regular maintenance treatments once the hair growth is minimized.

 | Psoriasis Study You may qualify for this study if you have been diagnosed with chronic moderate to severe plaque psoriasis by a medical doctor and are currently being treated with Raptiva or another biologic psoriasis treatment. Qualified participants will receive study-related medical evaluations at no cost. No medications will be provided as part of the study - all participants will continue with their current drug therapy. Reimbursement for your participation will be provided.
Psoriasis Study If you are aged 18 or older and have moderate to severe psoriasis, you may be eligible to participate in a new research study using Enbrel®, a drug that has been approved by the FDA that can be administered at home to treat the symptoms of psoriasis.
Eczema Study This study is enrolling patients ages 2-17 years old with atopic dermatitis (eczema) who have used Elidel for at least 6 weeks in the past 6 months (they do not need to currently be using Elidel). Participants will complete a short survey which will be mailed to your home every 6 months for the duration of the study. No office visits are required. Reimbursement for your participation will be provided for every completed survey.
Eczema Study Do you have eczema or atopic dermatitis? Have you used Protopic (tacrolimus) Ointment before you were 16 years of age to treat your eczema? Would you be interested in participating in a long-term program that will monitor your health? You may be eligible to participate in an observational research study. Skinfo® Product Study We are comparing Skinfo products to prescription and over the counter products. Subjects will be given two products, one Skinfo product and a comparable prescription or over the counter product, and will be asked to rate the two.
Arnica Study Patients having filler to their undereyes and/or marionette lines will be given a kit of an oral and topical medications to reduce the amount and duration of bruising from the treatment. Subjects will be asked to complete a questionnaire and photographs will be taken.
Restylane/Perlane Study We are looking for 10 subjects with moderate to severe aging of the face, especially under the eyes, the cheeks, nasolabial folds, and marionette lines. Subjects will receive one treatment of 4-6 syringes of Restylane and Perlane and will be followed for the 9 months following the treatment. Subjects will receive 50% off the usual fee for this treatment. Interested participants must be willing to have photographs and release them to Advanced Dermatology and SKINQRI. A cosmetic consultation needs to be performed prior to enrollment in this study.
